What Do I Need to Bring to an Appointment?
You will need the following:
- A valid PA identification.
- Medical documentation: for example - your doctor's list of your medical conditions and diagnoses, a medical summary, or doctor's note
If you don't have medical records, that is alright. We will perform a comprehensive medical history and evaluation.
Important Tips
For Initial Certification:
- Make sure the condition you want to treat is on the list of qualifying conditions in PA.
- After you register with the state, make an appointment with Dr. Villeneuve.
- After the doctor evaluates and certifies you for an alternative medicine card, return to the PA Dept of Health site to pay the state's $50 fee, and your card will come in the mail.
For Recertification:
- You need to renew your registration BEFORE the doctor can complete your certification:
- Make your appointment with Dr. Villeneuve.
Qualifying Conditions in PA
- Amyotrophic lateral sclerosis (ALS, "Lou Gehrig's disease")
- Anxiety disorders
- Autism
- Cancer (past or present)
- Chronic inflammatory demyelinating polyneuropathy (CIDP)
- Dyskinetic and spastic movement disorders
- Intractable spasticity (caused by damage to the spinal cord)
- Epilepsy / seizures
- Glaucoma
- HIV / AIDS
- Huntingtons disease
- Inflammatory bowel disease (ulcerative colitis and Crohns disease)
- Intractable seizures
- Multiple sclerosis
- Neurodegenerative diseases
- Neuropathies
- Opioid-use disorder
- Parkinsons disease
- Post-traumatic stress disorder (PTSD)
- Severe, chronic, or intractable pain
- Sickle cell anemia
- Terminal illness
- Tourettes syndrome (effective July 20, 2019)
